Kansas City continues its focus on integration and understanding social determinants of health and health-related social needs at the community level. Humana and its partners implemented the measures of Hunger Vital Signs, the UCLA Loneliness Scale, and Healthy Days into community-based settings. Additionally, Kansas City made progress in understanding the social health referral landscape and its complexities in the KC Metro, which led to the development of a Community Conversation platform to engage multidisciplinary stakeholders across the area.
