Making connections between clinical and social needs
Humana continued to build consensus on standards for screening of Social Determinants of Health (SDOH) in San Antonio. Humana reinforced those efforts by integrating with social health access referral platform Signify. The collaborative platform addresses both clinical and social needs for members. The challenges, stress and health concerns created by the COVID-19 pandemic on the community, providers and patients makes the work we are doing and the connection between clinical and social health factors more essential. Food insecurity continues to be the most significant and growing challenge. Humana continues to support the efforts of the San Antonio Food Bank to meet the needs of those most impacted by the pandemic.
